The Triage RN receives calls from Hospice patients and families, assesses the needs of the patient and family, and coordinates needed care over the phone, including scheduling home visits as needed. The Triage Nurse collaborates with Interdisciplinary Group (IDG) members and facilitates communication among the IDG and between Hospice and the patient’s Attending Physician. All services provided to the patient and families are pursuant to a physician’s orders and consistent with the patient’s Plan of Care. All communications and services are documented in the EMR in an accurate and timely manner. The Triage RN uses clinical judgement, knowledge and experience for guidance in the decision to either give self care advice, contact the on call clinician to visit the patient and/or contact the physician. Additionally, the Triage RN functions as the point of contact to the clinicians working during their shift.

  • Plans, directs, and participates in delivery of professional services to hospice clients. Focuses services toward diagnosis-specific client care needs.

  • Communicates significant findings, problems, changes in condition or environment to the physician and other personnel involved with patient care.

  • Reports unsafe conditions to the physician as appropriate.

  • Initiates plan for patient safety, using the patient, family, and community resources.

  • Demonstrates knowledge in all levels of hospice Nursing practice.

  • Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation.

  • Verifies the plan of care for the episode according to physician’s orders, assessment data, and established standards and guidelines.

  • Promotes the philosophy, mission statement and administrative policies of agency in an effort to ensure quality and consistency of patient care.

  • Coordinates a team of staff, to ensure the delivery of safe, effective, and appropriate hospice services.

  • Coordinates a team of staff to promote quality services for clients.

  • Coordinates services to ensure the availability of staff to respond to agency referrals and provide assessment visits within 24 hours of referral.

  • Establishes and implements priorities for services within the limitations of available resources.

  • Process after hours workflow to coordinate patient referrals, physician orders, staff assignments, patient re-certifications, and communications between various caregivers, the patient and their family, and the referring physician.

  • Remains available during all hours of the shift

  • Completes other duties as assigned during low call volume times.

  • Notifies managers immediately of unusual patient, family or customer situations.

  • Determines appropriate need for spiritual or social work services or other support services. facilitates visits as needed.

  • Demonstrates respect and courtesy with a positive and receptive attitude in all interactions with patients, families, and internal/external customers.

  • Assures compliance with the requirements of state licensure, Medicare certification, and any other applicable oversight agencies.

  • Promotes compliance with all fiscal intermediary and/or other third party payers, through education, coaching, and other assistance as necessary.

  • Maintains current knowledge of , and promotes compliance with all federal, state and local regulations.

  • Assures the continuity of quality patient care delivery.

  • Maintains communication with Clinical Supervisor providing updates and changes in schedules, patient data or needs, physician orders or special instructions.

  • Participates or communicates in patient care conferences offering clinical suggestions and updates of patient’s progress toward goals.

  • Assures clinical notes indicate continuing communication and coordination of services with physician, other staff and disciplines.

  • Demonstrates effective time management skills by completing call follow up within projected time frame.

  • Demonstrates commitment and professional growth.

  • Attends all required in-service programs within the review year as assigned by management. Independently obtains CEU’s as required by licensure and provides a copy of same to human resources department.

  • Promotes the Agency Philosophy, mission statement, and administrative policies to ensure quality of care.

  • Reports to work as scheduled.

  • Meets minimum productivity requirements based on agency standard.

  • Abides by company policy and procedures as outlined by company policy manual.

  • Performs other duties as assigned by the Supervisor.

  • Maintains patient & staff privacy and confidentiality at all times pursuant to HIPAA Privacy Final Rule.

  • Provides positive, supportive communication to physicians, patients, families, visitors, and other agency personnel.

  • Maintains availability by phone as directed by company policy.

Minimum Qualifications/Requirements:

EDUCATION: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. BSN preferred.

LICENSURE: Current State License as a Registered Nurse.

EXPERIENCE: 3-5 year’s experience as a registered nurse in an appropriate clinical care setting, and a minimum of 1-2 years of hospice experience required.

SKILLS: Nursing skills as defined by accepted nursing standards. Requires interpersonal skills and ability to manage patient care episodes. Ability to make quick decisions, multitask and handle stress; strong written and verbal communication skills; ability to work as part of a team; interpersonal skills; compassion

TRANSPORTATION: Reliable transportation.

HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.

Physical Requirements/Working Conditions:

  1. Complete required work on a daily basis.

  2. Cope with work involving ill, disabled, and dying patients.

  3. Sit, stand, bend, lift, and move intermittently.

  4. Move equipment and supplies necessary for job functions.

  5. Read, speak and write English in order to fulfill job functions in an understandable way.

  7. Meet the health requirements of the agency.

  8. May have possible exposure to infectious diseases.

  9. Abide by all occupational safety and bloodborne pathogen protocol to minimize illness and/or injury.

  10. Move, support and/or transfer patients as required to perform essential functions of position.
