The Clinical Supervisor is a registered nurse who manages the coordination, supervision, and implementation of professional and supportive services to Hospice clients in a safe, cost-effective manner. The Clinical Supervisor is responsible for the coordination of care during the episode of care. The Clinical Supervisor will provide education and training related to clinical practice issues and regulation and reimbursement changes with new employees during precepting phase of new hire orientation. On Call responsibilities and may act as the Hospice Administrator/back up administrator.
Plans, directs, and participates in delivery of professional services to home care clients. Participates in the organization’s strategic planning activities. Focuses services toward diagnosis-specific client care needs.
Communicates significant findings, problems, changes in condition or environment to the physician and other personnel involved with patient care.
Reports unsafe conditions to the physician as appropriate.
Initiates plan for patient safety, using the patient, family, and community resources.
Demonstrates knowledge in all levels of hospice Nursing practice.
Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation.
Verifies the plan of care for the episode according to physician’s orders, assessment data, and established standards and guidelines.
Promotes the philosophy, mission statementand administrative policies of agency in an effort to ensure quality and consistency of patient care.
Manages and directs a team of staff, to ensure the delivery of safe, effective, and appropriate hospice services.
Coordinates and directs staffing to promote quality services for clients.
Coordinates services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referral.
Provides education and training related to clinical practice issues and regulation and reimbursement changes.
Facilitates admission reviews and case manages for a team of patients.
Establishes and implements priorities for services within the limitations of available resources.
Supervises the coordination of services with providers in the delivery of care and related issues.
Process and coordinate patient referrals, physician orders, staff assignments, patient re-certifications, and communications between various caregivers, the patient and their family, and the referring physician.
Meets with staff on a regular basis to provide guidance and information related to specific issues.
Coordinates on-call services provide by hospice team.
Develops a working relationship with other health care professionals in the community and families to identify resources available and to ensure access of information to clients.
Supervises and provides clinical direction to the interdisciplinary hospice team & office staff to ensure quality and continuity of services provided.
Discusses and directs the field staff assignments for patient care. Compares the Home Health Aide care plan to the actual care given as verified with patient/caregiver.
Instructs and evaluates the field staff with special patient care procedures in accordance with the scope of practice and home health care policies of the agency.
Identifies and instructs staff on admission criteria for new clients and evaluates eligibility for home health care benefits.
Provides client and families with information about covered services.
Oversees monthly review of records to ensure continuing hospice care client eligibility for covered services.
Assures compliance with the requirements of s tate licensure, Medicare certification, and any other applicable oversight agencies through policy and procedure development, staff education, and ongoing monitoring activities.
Recommends new policies and/or changes in current policies to improve quality of client care andefficiency/effectiveness of services.
Promotes compliance with all fiscal intermediary and/or other third party payers, through education, coaching, and other assistance as necessary.
Maintains current knowledge of , and promotes compliance with all federal, state and local regulations.
Keeps fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance.
Supervises and provides direction to agency personnel in an effort to ensure quality and continuity of services.
Participates in the orientation of personnel.
Identifies the standards of practice for agency personnel and communicates these standards to them.
Provides staff development and in-service education programs in collaboration with the education department to ensure competencies of s taff and to meet regulatory requirements.
Provides leadership to clinical and office staff.
Communicates effectively with all members of t he interdisciplinary team.
Interprets and enforces human resource policies and procedures in a consistent manner.
Conducts performance evaluations in accordance with agency policy.
Monitors employee turnover, overtime and absenteeism. Takes appropriate actions to address problems/issues.
Collaborates with the Regional Director and other clinical management staff to ensure proper staffing of qualified, competent staff.
Administers, coordinates and evaluates operational Systems to enhance Operations and internal delivery of quality patient care.
Provides leadership to team and support staff in identifying agency/client needs and opportunities for Performance Improvement.
Assists with medical community education and discharge planning by participating in departmental meetings.
Assists quality improvement teams with data collection and documentation review for the Performance Improvement plan established by the agency.
Identifies and implements changes in clinical and/or operational practice based on the findings of the Performance Improvement program.

Provides in-service education programs in collaboration with the education department to ensure competencies of staff to meet regulatory requirements.
Identify educational needs for field staff & office staff.
Pursues ongoing education to remain current with all aspects of profession.
Facilitates problem-solving sessions with nursing and all staff applicable to resolve client and/or reimbursement source issues.
Stays current on the available community resources, health care costs, and industry trends through self- education and access t o outside educational opportunities.
Provides support and direction to agency staff, other health care professionals, clients, and families related to appropriate and available health care resources.
Implements and performs liaison activities with both individual clients and the community as a whole.
Responsible for branch facility, including but not limited to communicating IT and facility maintenance needs and assuring professional appearance, HIPAA compliance including up to date required postings (license etc)
Promotes a safe environment for clients and co-workers as well as personal safety.
Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements.
Maintains confidentiality in all aspects of the job.
Assess safety; recognizes and responds appropriately to potentially unsafe situations.
Monitors assigned cases to ensure compliance with requirements of third party payor.
Ensures that all assessments,visitnotes, supplyrequisitionsand time cards are processed timely and accurately according to office policy.
Demonstrates knowledge of current status of requirements of third party payor, by identifying and reporting potential payment/coverage problems. (This includes but is not limited to Medicare, Blue Cross/Blue Shield, Medicaid and other 3rd party companies.)

Attends all required in-service program within the review year as assigned.Independently obtains CEU’s as required by licensure and provides a copy of same to human resources department.
Promotes the Agency philosophy, mission statement and administrative policies to ensure quality of care.
Maintains patient & staff privacy and confidentiality at all times pursuant to the HIPAA Privacy Final Rule.
Maintains availability by phone as directed by company policy.
Promotes Agency image by adhering to the dress code.
Reports to work as scheduled.
Meets minimum productivity requirements based on agency standard.
Abides by company policy and procedures as outlined by company policy manual.
Performs other duties as assigned by leadership
EDUCATION: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. BSN preferred.
LICENSURE: Current State License as a Registered Nurse.
EXPERIENCE: 2-3 years of supervisory experience as a registered nurse in an appropriate clinical care setting,
home health or hospice environment preferred. Evidence of clinical and supervisory knowledge and experience
appropriate to the assigned supervision responsibilities.
SKILLS: Nursing skills as defined by accepted nursing standards. Requires interpersonal skills and ability to manage patient care episodes.
TRANSPORTATION: Reliable transportation.
HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.
Physical Requirements/Working Conditions:
