Hospice Registered Nurse (RN) Case Manager, Weekend On Call
Screen referral for appropriateness and performadmission to facilitate the transition of the patient and family into thehospice program. Also responsible to Case Manage patients and families toensure symptom management, patient and family education, medication management,coordination of hospice services and provide excellent communication with thepatient’s physician.
Coordinatesthe interdisciplinary care team for assigned patients, and is designated tocoordinate the implementation of the plan of care.
Providesthe primary nursing care for hospice patients, regardless of the setting.
Assumesfull responsibility for professional management of assigned hospice patients’care related to the patient’s terminal illness.
Teachesthe primary caregiver skills that are needed to care for the patient in theirresidence.
Worksclosely with the attending physician and other team members to developappropriate pain and symptom management.
Completesnursing assessments and admissions of patients.
Maintainsthe clinical record to meet federal, state, and agency regulations andguidelines.
Superviseshospice aides and certified nurse aides, making a supervisory visit at leastevery 14 days, and face to face at least monthly.
Assumesall on-call responsibilities as assigned.
Participatesin Performance Improvement Program.
Participatesin and presents inservice programs, team conferences, and staff meetings.
Providesskilled nursing care to assigned patients/families as outlined in the plan ofcare.
Participatesin the development of the patient/family interdisciplinary care plan.
Forpatients residing in a skilled nursing facility, the RN Case Manager will haveresponsibility for managing the patients’ symptoms related to the terminalillness.
Verifyby patients visit and review of documentation that the caregivers in the SNF arecarrying out the plan of care as written.
Haveultimate responsibility for developing and integrating the plan of care withthe hospice team and the SNF team to assure one integrated plan of care thatreflects the hospice philosophy.
Forpatients receiving private home care services, the RN Case manager shallsupervise the certified nursing aide, sitters, and companion services assignedto patients.
Performother duties as assigned by the DHS or the Administrator.
KNOWLEDGE, SKILLS, ABILITIES:
Athorough understanding of Hospice Regulations (federal, state, and localgovernment).
Possessexcellent communication skills, both oral and written.
Abilityto handle confidential matters.
Abilityto work a flexible schedule.
Commitmentto the Hospice concept and standards.
Abilityto interact with a variety of different age groups.
Participatein agency surveys and any subsequently required reports.
Attendand participate in continuing educational programs to keep abreast of changesin your field as well as to maintain current license/certification, asrequired.
Attendand participate in mandatory inservices.

Complywith corporate compliance program.
Reportjob-related functions/tasks that involve occupational hazards includingexposure to blood and body fluids and others as necessary.
Followestablished safety regulations, to include fire protection & prevention,smoking regulations, infection control, etc.
Followestablished safety procedures when performing tasks and/or working withequipment.
Performother related duties as necessary and as directed by supervisor.
MINIMUM EDUCATION REQUIRED:
Minimum H.S. diploma or equivalent.
MINIMUM EXPERIENCE REQUIRED:
A minimum of three (3) years of nursing experience is desirable.
Hospice or home care experience is preferred.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Currently licensed as an RN in the states where you will provide care.
Must have current driver’s license and valid automobile liability insurance.
Must have certification in basic life support CPR.
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Ability to work effectively independently and within an interdisciplinary team
Orientation to Hospice and any other training that may be necessary to complete the assigned duties effectively.
Communicates well with patients/residents/clients and family members providing warm and friendly greeting and an approachable attitude to families, visitors, patients/residents/clients and responds to expressed concerns while displaying a helpful, caring demeanor.Answers questions when appropriate in a professional manner.
